Please attest that your student tested negatively for Covid-19 on our Absentee Verification Form when your child returns to school following an absence due to:
- Fever or chills
- Cough
- Shortness of breath or difficulty breathing
- Fatigue
- Muscle or body aches
- Headache
- New loss of taste or smell
- Sore throat
- Congestion or runny nose
- Nausea or vomiting
- Diarrhea
- For other reasons/symptoms: Please fill out and send in the Absentee Verification Form when your student returns to school
